Horizon Peers is a New York–based staffing agency that connects certified peer support professionals with organizations supporting individuals facing mental health challenges. Our trained, NY State–certified peers offer recovery-focused guidance, helping clients reintegrate into the community, achieve personal goals, and feel truly understood — one meaningful connection at a time.

Home and Community-Based Services (HCBS).

At Horizon Peers, we go beyond traditional support by offering specialized services for children with medical fragility or emotional disturbances through Home and Community-Based Services (HCBS).

Families are empowered to choose a Direct Support Professional (DSP) who works closely with them to implement personalized support plans, including:

  • In-home care

  • Community engagement

  • Respite services

  • And other essential supports

Our goal is to foster each child’s independence, well-being, and quality of life in a compassionate and structured environment.

Care Management Services

We also provide Care Management Services for individuals facing chronic medical or mental health challenges. Our dedicated care managers help by:

Coordinating healthcare and related services

  • Creating individualized care plans

  • Advocating for access to necessary resources

  • Offering emotional and practical support

This service helps bridge the gap between individuals and the care they deserve, ensuring a seamless and compassionate support experience.

Peer Certifications We Accept

We work with a wide range of peer certifications recognized across New York State. If you hold one of the credentials below, you may qualify for job placement through Horizon Peers.

  • Adult Peer (NYCPS)
    Certified to work with adult clients aged 18 and above.
  • Youth Peer (YPA)
    Certified to work with youth clients ranging from ages 0 to 21.
  • Family Peer (FPA)
    Certified to support the caregivers of youth clients. Services are billed under the child’s insurance.
  • OASAS Peer (CRPA)
    Certified to work with adults. This certification is temporarily approved by OMH, allowing CRPA-certified peers to work with our clients while they work toward obtaining the NYCPS certification.
